Order Form Instructions

Please follow the instructions for each section exactly.  These instructions are for our benefit, as well as yours, and are intended to expedite your order.  Failure to fill out the form properly could lead to confusion on your order and unnecessary delays.  At any time, if you have questions, you may call us at (717) 986-1791, or email us at customerservice@novoselenterprises.com for assistance with your order.

Section #1 (Customer Information):
Please fill out all fields including a phone number.  If you've ordered from us before, please enter your customer # that was on your previous invoices.  This section is for the person/s who is/are actually placing the order.  Even if you are purchasing this order for another individual or business, your information is needed here so we know who is placing and paying for the order.  You will have the chance to input a different shipping address in the next section.  Your email address is optional but inputting it will allow us to inform you when your order has been processed and shipped.  A phone # is required for all mail-in and faxed orders.

Section #2 (Shipping Information):
Please use this section if the order is being shipped to an address other than your own.  Possibilities include shipments to businesses, family members, or orders intended as a gift for someone else.  This is the address that will actually be placed on the package for delivery.  If the shipping address is the same as your own address, then please write "Same as Above" across the entire section and then move onto the next section.

Section #3 (Order information)

This section can be filled out exactly as the information appears in our shopping cart.   All you need to do is add the items you wish to the cart and the cart will automatically apply all applicable discounts and give you the product subtotal.  Once you enter your zip code, the cart will then display the shipping rates for the order, to your location and present you with the order total.

Please include the order total in either a Check, Money Order, or Credit Card information and mail the form and your payment to address at the top of the form.
